Phendo is a research app from Columbia University Department of Biomedical Informatics (IRB-AAAQ9812). It is designed to transform the way individuals living with endometriosis manage their condition, while contributing data to advance scientific research on this disease. Phendo is part of the Citizen Endo project. Email citizenendo@columbia.edu for any questions about Phendo or our endometriosis research.

This consent and HIPAA authorization form is for the Phendo and Medical Record sub-study. You can participate in the main study without participating in the sub-study (sharing your medical records) by downloading the Phendo app on the App Store or Google Play.

Access to your health information (i.e. medical records) is required to be part of this sub-study. If you choose to take part in this sub-study, you are giving us the authorization (i.e. your permission) to use the protected health information and information collected during the research that can identify you.  The health information that we may collect and use for this research may include medical history that may be considered sensitive. Information about you may be obtained from any hospital, doctor, and other health care provider involved in your care that is needed for this research purpose. Your authorization to use your health information will expire when the research is completed.
